Medication-Assisted Treatment (MAT)
MAT combines clinically validated medications with psychotherapy, making it one of the most effective comprehensive approaches for treating alcohol addiction.
What is Medication-Assisted Treatment?
MAT is not "replacing one drug with another." It uses medications to restore the brain's chemical balance disrupted by alcohol, reducing withdrawal symptoms and cravings, enabling patients to better engage in psychotherapy and recovery activities. Medication must be combined with psychotherapy for optimal results.
Detoxification Phase Medications
Benzodiazepines
Representative drugs: Diazepam (Valium), Clonazepam, Lorazepam
Mechanism: Acts on the same GABA receptors as alcohol, safely replacing alcohol's inhibitory effects to prevent nervous system overexcitation
Use: Controls withdrawal symptoms, prevents seizures and delirium tremens
Note: Only for short-term use during detox due to their own addiction risk; not for long-term treatment
Maintenance Treatment Medications
Naltrexone
Effect: Blocks opioid receptors in the brain, reducing pleasure from alcohol
Outcome: Decreases drinking motivation and cravings
Dosage: Daily oral or monthly injection (extended-release Vivitrol)
For: Long-term maintenance after detox completion
Acamprosate
Effect: Regulates balance between glutamate and GABA systems in the brain
Outcome: Reduces post-abstinence discomfort and cravings
Dosage: Oral, three times daily
For: Patients who completed detox and wish to maintain sobriety
Disulfiram (Antabuse)
Effect: Inhibits acetaldehyde dehydrogenase, causing acetaldehyde buildup after drinking
Outcome: Produces aversive reactions (nausea, vomiting, headache, palpitations) when alcohol is consumed
Dosage: Daily oral
For: Highly motivated patients needing external deterrent
Adjunctive Medications
- Vitamin B1 (Thiamine): Prevents/treats Wernicke's encephalopathy; administered to all patients upon admission
- Folate & Multivitamins: Corrects nutritional deficiencies from chronic alcohol use
- Gabapentin: Helps control anxiety and insomnia, alleviates withdrawal discomfort
- Antidepressants/Anxiolytics: Treats comorbid mental health conditions (requires post-detox evaluation)

- All medications must be used under professional medical supervision—never self-prescribe or adjust dosages
